A Typical Case of Paradox Cerebral Embolism during Cemented Total Hip Arthroplasty

ثبت نشده
چکیده

The procedure was performed under general anaesthesia. Both cup and stem were cemented. While implanting the cemented stem the heart beat raised from 60bpm to 95bpm and the blood pressure dropped to 90/45mmHg (Figure 2). To the end of the procedure the patient had recovered and was extubated showing stable vitals. After surgery a slurred speech and a deviation of the eyes were observed. The MRI (1,5 T, T1w SE sagittal, FLAIR coronar, T2w TSE transversal, bleedingand diffusions sensitive sequences T2w TSE sagittal TOF angiography and secondary multiplanar reconstructions) showed multiple ischemic regions in the dorsal flow area of the cerebellum on both sides, especially in the pica flow area, the left thalamus, left temporal and occipital due to multiple embolisms (Figure 3). Space occupying cerebral edema was not found.

Intraoperative cerebral arterial embolization during total hip arthroplasty.

BACKGROUND Fat embolism to the pulmonary circulation is known to occur during total hip arthroplasty, especially during insertion of a cemented femoral component. Fat and air bubbles may enter the systemic circulation via a patent foramen ovale or through pulmonary circulation. METHODS To determine whether microemboli to the brain were occurring during total hip arthroplasty, 23 patients unde...

Hip Arthroplasty and its Revision in a Child: Case Report and Literature Review

Juvenile idiopathic arthritis is the leading cause of hip replacement in young children. However, arthroplasty in this population is challenging with several concerns about quality of the growing bone, young age for revision surgery, and difficulties in potential several revisions. In this study we introduce a case of a 12-year old who is one of the youngest patients to undergo revision hip art...

The frequency of venous thrombosis in cemented and non-cemented hip arthroplasty.

We randomised 250 patients undergoing unilateral, elective hip arthroplasty for osteoarthritis to receive either a cemented or a non-cemented Mallory Head prosthesis. Aspirin was used as prophylaxis against thromboembolism during the first half of the study and adjusted-dose warfarin during the second half. Postoperatively, all patients were asked to have bilateral venography and 80% agreed. Al...

Paradoxical fat embolism after uncemented total hip arthroplasty: a case report.

The authors report a paradoxical presentation of fat embolism after uncemented total hip arthroplasty. The patient presented vertigo and diplopia after surgery. Cerebral fat embolism was diagnosed by MRI. A patent foramen ovale was responsible for the venous to arterial circulation shunt. Treatment was conservative. Spontaneous and complete recovery occurred.

Effectiveness of the ROBODOC system in preventing intraoperative pulmonary embolism.

Intraoperative pulmonary embolism occurs not only during cemented but also during cementless total hip arthroplasty (THA). We determined whether the ROBODOC femoral milling system can reduce intraoperative pulmonary embolism, by using of transesophageal echocardiography and hemodynamic monitoring. We did a prospective clinical trial with 71 patients (75 hips) who were divided into 2 groups: gro...
